Upon completion of the of this learning activity at least 85% of attendees will have gained knowledge of how patient navigators recognize and address barriers experienced by patients with limited literacy skills when accessing health information and services, of how patient navigation practices respond to unique institutional environments and patient populations, and about the origins of patient navigation in relationship to addressing Social Determinants of Health.
2.0 Contact Hours of Nursing Continuing Professional Development (NCPD) will be awarded for attending the program in its entirety and completing an evaluation.
